How to File a Grievance
The Thriving Mind Continuous Quality Improvement Department follows up on all complaints and grievances received on behalf of a Network Service Provider. Our contracted providers are listed here.
Thriving Mind values the concerns of the public and offers a process by which to express and seek resolution to issues and concerns concerning any of its contracted network providers.
Thriving Mind seeks to ensure that complaints and grievances are handled and resolved in an appropriate, fair, transparent and timely manner. No person shall be subjected to discrimination or adverse treatment for participating in a complaint or grievance.
Formal complaints and grievances may be submitted verbally or in writing and may be withdrawn by the filing person at any stage in the process by proving verbal or written notice. Confidentiality shall be maintained to the extent possible and in accordance applicable law(s).
Any complaint involving allegations requiring notification to a regulatory authority will be immediately reported. This includes, but is not limited to the Abuse Hotline, the Office of the Inspector General (OIG), the Florida Department of Health (DOH), the Department of Children and Families (“The Department”), and Health and Human Services (HHS).
