Our Commitment
We are committed to helping individuals with mental health and/or co-occurring conditions access safe, affordable, and stable housing, along with the support services they need to succeed in the community. We also seek to increase discharges from state mental health treatment facilities to stable community housing, reducing discharges to crisis stabilization units, addiction receiving facilities, or placements that increase the risk of homelessness. While we work with community partners to improve access to housing, we do not manage, operate, or license housing facilities.
Need housing assistance?
If you have questions regarding employment and housing opportunities, please ask the case manager or other staff at the community agency from which you are receiving services. Many of the mental health and/or substance abuse agencies have programs which may begin with sobriety and medication management and then progress to employment and housing assistance. If you are not linked to a community mental health center, refer to the section titled “Community Mental Health Centers (CMHC)” in this manual.
Supportive Housing/Living
Supported housing/living services assist persons with substance abuse and mental health issues in the selection of housing of their choice. These services also provide the necessary services and supports to assure their continued transitioning and successful living in the community.
Assisted Living Facility (ALF) Licensure & Unlicensed Activity Resources
Thriving Mind South Florida encourages network providers and others coordinating services or making referrals to Assisted Living Facilities (ALFs) to verify that facilities are appropriately licensed by the State of Florida.
The Florida Agency for Health Care Administration (AHCA) provides resources to verify facility licensure and identify or report potential unlicensed activity.
Verify an Assisted Living Facility License
Before making a referral or placement, use AHCA’s FloridaHealthFinder Facility Locator to search for a facility and verify its current licensure information.
Identify or Report Potential Unlicensed Activity
If you have concerns that an Assisted Living Facility or other health care provider may be operating without the appropriate license, AHCA provides information about how to recognize and report potential unlicensed activity.

Why Licensure Verification Matters
Verifying licensure helps ensure that individuals are referred to facilities operating under applicable State of Florida licensing requirements.
Providers should verify a facility’s current license status directly through AHCA when considering a referral or placement.
